Hairline lowering surgier was in this 46yo female an extremely effective way to return her hairline that had considerably receded due to the condition of frontal fibrosing alopecia (FFA) to a much more aesthetic position. In FFA, hair transplantation is oftentimes not effective, however in addition to the surgery to lower the hairline, it is possible to do a fat transfer to the sideburn/temples areas to then allow hair grafting of these areas to be successfully performed.

For a man to be a good candidate for this surgery, the scalp needs to be flexible, and the hairline stable. The SHA provides a denser hairline than what can typically be achieved with hair grafting.

This male patient had significant scarring from prior poor grafting as can be shown in the frontal closeup. In order to fully repair this situation, he underwent a complete hairline excision, whereby the original nontransplanted hairline was advanced forward allowing for the removal of the scarred skin with prior transplants, resulting in a fine-line scar that will at some point be grafted to soften it. The removed grafts were dissected out from the scar tissue and planted into the old donor site scars.

In order to restore the elevated hairline that was the result of frontal fibrosing alopecia (FFA), this patient underwent hairline advancement surgery. This involved advancing her unaffected hairline forward, permitting the removal of the scarred frontal scalp. Because she had poor scalp mobility, in this case a balloon expander was used.
